Ideally you should be:

  • Dedicated, enthusiastic and passionate about what you do

The skills and experience we are looking for:

  • An RGN with a minimum of 3 years post-graduation experience

  • Conversant with the NMC code and CQC fundamental standards and regulatory requirements

  • Experience of working with end of life clients / palliative care

  • An understanding of the community care setting

  • Excellent written and verbal communication skills

  • An ability to complete risk assessments and outcome based care plans

  • Some experience of working with clients with complex clinical needs e.g. tracheostomy, PEG, Non-invasive ventilation

  • Existing relationships with or confidence to build relationships with Clinical Commissioning Groups and other MDT members

  • A creative and innovative thinker who strives for excellence and has the passion to mentor and motivate other staff

  • A person who loves being a team player as much as they do leading a team


The role will require you to:      

  • Manage client care packages, monitor client care and well being

  • Complete and review Client Needs and Risk Assessments in line with CQC guidance

  • Develop, review and implement Client Personal Care Plans in line with CQC guidance

  • Liaise with Social Services and attend case conferences/ MDT meetings as required

  • Work with the Training Team and assisting with clinical training where possible

  • Inform the Branch Manager/Operations Manager/Clinical Governance Manager of any issues that are likely to adversely impact quality of client care and /or health and safety of employees.
