Your care is delivered by a Routes Healthcare team who are vetted, trained and supervised within our Governance & Clinical Framework. Our quality standards are overseen by our Registered Managers, Nurses and Service Improvement Team, and of course regulated by the Care Quality Commission (CQC).

Consistent plan reviews
Your needs, goals and capabilities change constantly.
That’s why we consistently review your care plan, with a formal overview every 6 months.
To include:
- Follow up actions and referrals
- Escalate when necessary to commissioners and review notifications to Health & Social Care System
- Measure outcomes
- Ensure any changed are made
- Continue to deliver safe and effective care
Gathering regular feedback
Our team are always striving to deliver a better level of care tomorrow, than they did today.
We are keen to get as much feedback as possible from you and your support network.
This allows us to respond effectively and efficiently and avoid any unnecessary distress.
Specialist care teams
Many people come to us with fast-progressing and debilitating conditions that require expert care.
At Routes, we match you to a care team who are experts in specialist treatment for certain illnesses. This leads to a noticeable difference in anxiety, levels of sleep, number of bumps and falls and personal relationships.
This is because we can pre-empt movements and reactions, spot trends in triggers and reduce demands on other services that would be necessary if we weren’t experts.
