Complex Care

We are highly experienced in delivering a range of services, which are individually tailored to support people to live independently at home and within the wider community. We are passionate about making a positive difference to individuals lives and placing the person at the centre of everything we do. Our complex care team, includes a mixture of Case Managers and Nurses to help support people with everything from specific clinical interventions to planning a holiday.

End of Life Care

We provide care for adults with life limiting illnesses and aim to provide specialist palliative care to the highest standard, consisting of symptom management, emotional support and end of life care. We are immensely privileged to deliver a service which actually affirms and improves quality of life by providing individuals with relief from symptoms, pain and stress, enabling their dignity and supporting their loved ones through the highs and lows of their illness and beyond.

Clinical Support

Our Clinical Support Service team are uniquely positioned and experienced in designing and implementing dedicated clinical pathways to ease pressure on existing District and Community Nursing teams, freeing up valuable Nursing capacity within the Health & Social Care system.

System Solutions

We specialise in building tailor made and dedicated services, both interim and recurrent, to meet capacity and patient flow concerns. With in depth sector knowledge, system understanding and local presence we mobilise teams to deliver, affordable and sustainable services at speed.
