You’re in safe hands
No one’s care needs or life goals are the same.
Each care plan we create is designed in collaboration with our client and those people important to them, where consented. From daily routines and personal requirements to partaking in their hobbies and holidays, we consistently review and set goals to support a client’s independence.
Our team consists of a variety of professionals, offering clients a balance of clinical oversight by registered nurses, with other professionals that offer either therapeutic approaches or social care experience. This Multi-Disciplinary approach means that we offer a truly holistic service whereby we can safely manage a wide range of clinical interventions and complex health condition along with peoples’ other needs and desires, for an independent life where each client has control over their lifestyle and personal choices.

Underpinned by Specialists
At Routes, we believe in innovation and constant development to consistently deliver better levels of care. To do this, we employee a range of professionals who are specialists in different areas of health and social care, that review and adapt services, including training programmes, to guarantee our care team can meet the personal requirements of each individual client.
Each client has their own designated team, which includes access to Nurses and Case Managers that have a range of different expertise backgrounds. This is to ensure each client receives a service that’s individual and unique to them and supported by relevant expertise.
Routes has a robust governance framework to ensure that any clinical/specialist procedures that are undertaken are carried out safely and in line with best practice and relevant guidance.
An early intervention approach
We utilise an early intervention and prevention approach to planning out care.
This helps us utilise a specially developed care plan that provides goals and outcomes that empowers our clients to attain and maintain their independence.
It’s what provides us with a clear set of actions that support positive and calculated risk-taking. So, our clients can live life to the fullest – in the safest way possible.
Consistent plan reviews
A client’s needs, goals and abilities can change over time.
Therefore, we always get to know our clients on a personal level and partner with their wider Health and Social Care system. This can include occupational therapists, physiotherapists and speech, language and movement therapists, and more to keep all elements of their needs and abilities in mind.
Let Routes be part of the journey
Choosing a care provider in these crucial moments is no easy decision. But with Routes, our clients get a team of superheroes who want to be there for our clients and their loved ones.
