Your Initial Call
Tell us about your client's needs, the timeline you're working to, and any concerns you have. We'll be honest about whether we're the right fit, realistic timescales, and how we'd approach the support.

2. Clinical Assessment
Our registered nurse meets your client at hospital, rehab unit, or home. We assess clinical needs, daily routines, communication abilities, and psychological state. You receive a comprehensive report with proposed care plan, staffing approach, and detailed costs.

- Full clinical review with medical history
- Moving and handling needs
- Communication and cognitive function
- Home environment and adaptations
- Psychosocial factors
- Risk assessments and safety planning

Support Team Recruitment and Training
We match your client with Support Workers who have the right clinical skills and temperament. Every team member is Routes-employed, DBS checked, and trained through Routes Academy with bespoke training for your client's specific condition.
Our approach:
- Recruit for clinical competence and personal compatibility
- Specialist training (tracheostomy, ventilation, PEG, challenging behaviour)
- Supervised introductions with your client
- Gradual settling-in period with nursing oversight
- Ongoing clinical supervision and competency assessments
Support Package Implementation
Once you're ready to proceed, we move quickly. Rotas planned, equipment coordinated, family liaison established. Our registered nurses supervise the first few days closely, making adjustments to ensure everything works smoothly from the start.

Ongoing Support and Reviews

- Updated care plans and risk assessments
